ADMISSION POLICIES

Admission Packet Information in the child’s record will include:

1. Child’s full name, birth date, home and mailing address and phone number.
2. Names of parents or guardians, parents or guardians’ current places of employment, addresses and work telephone numbers.
3. Name, address and phone number of two people who can assume responsibility for the child if the parent cannot be reached in an emergency.
4. Signed authorization to the effect that the parent gives permission to the Ark to seek transportation and medical treatment in an emergency situation when the parent cannot be reached or is delayed in arriving.
5. Name, address and phone number of the health care source to be called in case of emergency.
6. Special dietary needs:  Modification or allergy to certain foods.
7. Special medical conditions or allergies.
8. A release for the Ark nurse to review their immunizations, if needed.
9. External medication permission form.
10. Permission to participate in the Ark activities and written permission to participate in off-site activities.
11. Signed contract.
12. Date the child first attended the Ark and the date of the child’s last day at the Ark.
13. A copy of the up to date immunization record or Public Health Division approved exemption form the requirement.
14. A record of any accidents, injuries, or illnesses that require first aid or medical attention and any other observations of recent bruises, bites, or signs of abuse or neglect.

A record of the time the child arrived and left the facility and dates of attendance initialed by a parent, guardian, or person authorized to pick up the child.
